Actually, you are conjugating il, the "thing" that is going on, so all you would need to change is the object (recipient) to vous : Que vous arrive-t-il ?

Also, I would translate it more like "What is happening with you?", in the present tense. What happened to you (past) would be more like:

Que vous est-il arrivé ? (inversion of passé composé: il est arrivé)
