Well I think I have figures it out after going back to older lessons and resources on the type of verb Succedere is. It is what happened to you, not what you happened. It, therefore è, is the one doing the action, You are the object.
Right?!

Thanks for your question! Your reasoning is absolutely right - it refers to what happened to you/him/her/them/me, hence the verb is always “è”.
E.g.
Cosa ti è successo? -→ What happened to you?
Cosa vi è successo? -→ What happened to you (plural)?
Cosa mi è successo? -→ What happened to me?
