Are there any accurate translators online? I know google and other translator apps just translate the words, which means your sentences don't make sense?

The best translator is probably here - http://www.spanishdict.com/
It gives you translations by three different translators. With that said, I would not rely on it completely. Translators have trouble getting grammar right. I sometimes use it go determine what specific words mean (like a dictionary), and I also use it for its conjugation information, but I don't rely on it for translations.

Well, first of all, you are speaking."Como estoy?' "How am I?" I would think this might be a response to someone asking "¿Como estás?" or "¿Como está?" Someone asks you "How are you? You respond: "How am I? I guess I'm fine." Or you ask a doctor "¿Como estoy doctor?" "How am I doctor?" It's directed to the person you are speaking with and as I speak to myself in Spanish that person is often me.

Spanishdict.com is my absolute favorite, and does a good job of translating even long sentences most of the time. That being said, it still isn't perfect, but most of the time at least one of the 3 translations will be good.
The other thing I like is their verb conjugations (very complete and accurate) and sample phrases and answers from their forums. Somewhere on that site you should be able to find just about everything.
